After the first or the second session, you are required to complete the assessment and the disclaimer forms. The disclaimer form must be notarized. That is all there is to it. You decide when to stop the sessions.
Some professing Christians have become offended at the intensity of early sessions, particularly because we search for evidence of whether or not the person is born again. Our approach is scriptural, as Paul wrote to the Corinthians to examine and test themselves to see if they be in the faith. Each and every question on the DAF does not necessarily indicate bondage, but the DAF is helpful in guiding the counseling process by revealing strongholds that may have opened the door to demons claiming legal grounds to you. Those who are in serious torment want immediate relief. Since deliverance is a field that is oftened hindered by unknown factors, clients who seek and need deliverance MUST sign a Deliverance Disclaimer Form, taking full responsibility for "the casting out of demons."Please be advised that I do NOT routinely accept as clients those who are actively engaged within the mental health system and/or those who are ACTIVELY on medication prescribed by a psychiatrist for a mental health condition. I select clients from this group on a case by case basis.
In those rare cases with serious mental health issues that I DO accept, the client must sign an advised consent form which basically admits that he or she has been informed that the casting out of demons is based upon a mandate from Jesus Christ, a procedure which may not be considered an un-acceptable form of practice by the fields of psychiatry, psychology and social work. In this regard, the client must take full responsiblity and covenant not to sue.

Your counseling sessions with me are strictly confidential. If for the sake of your progress in counseling it would help for me to speak to a significant other, you should download and complete this form and have it notarized and mailed back to me. I will only consult with pastors, church members, relatives or other non- counseling professionals in a conference call, with you being able to hear what the other party is saying concerning you.
IF YOU ARE AN ACTIVE CLIENT IN THE MENTAL HEALTH SYSTEM.
If you have no mental health history, you may only need A FEW MONTHS OF telephone sessions each of one hour duration to find and demobilize the CAUSE. You should be aware of the fact that I generally do not accept clients who are actively engaged in mental health counseling within the secular system for several reasons. The main reason is that maintaining a counseling relationship with more than one counselor of differing opinions and approaches can lead to client confusion.
However, I decide on a case by case basis as I do not routinely turn away those who are on mental health medication. If the client is without psychosis---in touch with reality---I will accept clients diagnosed with a personality disorder, ie. narcissistic, borderline, etc. I also accept those who manifest blasphemous thoughts, are experiencing various forms of torment, including but not limited to spontaneous orgasm, panic disorder, obsessive compulsive disorder, excessive religiosity through charismatic witchcraft, the Jezebel spirit, auditory and visual hallucinations without a mental diagnosis of schizophrenia. I will most likely NOT accept a diagnosed schizophrenic who has an open mental health case.
In every case and situation that I Do accept the case, the client and I will seek together to uncover the truth. If you are not on mental health medication, the counseling duration may require less sessions than for a person who is on medication. I NEVER advise a client to not take prescribed medication. In fact, medication can be a helpful asset. My decision to accept a mental health case depends on various factors and other variables. If I accept a case where there is a mental health history, the duration of your counseling will certainly be extended beyond my ability to estimate early on.
